Before submitting your
claim, please take a moment to review the
following:
- Please make sure to fill out the
fields marked with asterisks; these are required fields and the
form will not submit properly if they are not filled out.
- Claim must be submitted in
writing or emailed; please contact your move counselor to find
out how long you have to file your claim.
- Please provide a detailed description of the
damages.
- Prompt settlement requires:
- Age of item
- Original cost
- Replacement cost
- An estimate of repair is NOT needed.
*Please do not discard damaged cartons, and have all
broken items available for inspection.
*All damaged items must be inspected by the carrier
prior to being moved again.
*Please do not attempt to repair items before the
carrier has the opportunity to inspect the damages.
